Our weight loss program needs to follow a very strict protocol, this will insure your success. It requires constant supervision of your individualized weight loss program. Keeping appointments and following instructions is a necessity. The medication should be taken as prescribed and only changed after consulting with our physician. The success of your weight loss during our program can be limited if you chose not to take the medication as prescribed.

Even though the weight loss program medication is very safe it should not be combined with other appetite suppressants or any other non-prescribed stimulating agents. Consult with our physician prior to taking any other medications for weight loss, or if you have any questions regarding drug interactions. We assume that you are not taking other medications for weight loss if you do not disclose them to us during your initial or follow up visits. We also assume that you are not getting these medications from other physicians or any other sources.

Sharing medications with friends or family members is absolutely forbidden and can be extremely dangerous, since these weight loss medications can have severe side effect if certain medical conditions are present.

Fee Schedule:

Patients commitment to weekly evaluations are required. We found this to be the key ingredient to the success of your weight loss program. In you have to be away for extended periods of time, we will give you enough medication to continue on the weight loss program, and will be available to you during this period. You will be charged for each week during your time away.

Charges are dependent on the complexity of the medical service provided, for this reason if an additional therapeutic agent is needed to enhance the success of your weight loss program, you will incur an additional charge.

Program payment is due prior to your visit. Our weight loss program is self-pay only we do not submit claims to any insurance company. We will not be able to see you If you are unable to pay for your visit in advance.

Our charges are as follows:

Weight loss Program Initial visit: $150 (unless you have an EKG and lab within 6 months)

Program Weekly Follow Up: $50

Additional MIC/B12 Combo Inject: $20

B-6 Injections: $20

B-12 Injections: $20

Multivitamins: $20

Calcium Pyruvate $25

Payments for our weight loss program are due when service is rendered and program products are purchased.
